v0.1.1: hedge maze and void fall bugfixes

Hedge Maze (Level 84):

• Increase wall_open_chance from 55% to 65%
• Add spawn dampening: 80% open at center, lerping to 65% over 5 cells (30 blocks), so spawn area is easier to navigate
• New config options: spawn_dampen_radius, spawn_dampen_chance

Fixes:

• Void fall entry now targets Level 0 (was incorrectly sending players to The Library)

Docs:

• Add detailed README with level descriptions, entry/exit conditions, entity types, behaviors, events, escalation, commands, permissions, and configuration reference
• Add screenshot assets and percolation test page
